Submission of this form serves as a formal agreement between the Producing Member Company and EMACT that the production(s) below be included for consideration in the DASH program and annual award consideration. Application and payment assures award consideration but does not guarantee nomination or receipt of an award.

  For all groups within a 30 miles radius of Boston the cost is $30 per production. Due to increase cost of gas and the distance of some of groups for the reviewers to travel groups beyond the 30 mile radius cost is $40 per production.
